Mental Health Rehabilitative Services

Mental Health Rehabilitative Services are designed to help individuals maintain or improve their functional abilities. These services are considered rehabilitative when they are necessary to assist a client in achieving specific rehabilitation goals outlined in their treatment plan. Mental Health Rehabilitative Services may be provided to individuals diagnosed with a serious mental illness, as defined in the latest edition of the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders (DSM).

These services are age-appropriate, personalized, and aimed at addressing functional impairments that negatively affect:

  • Community Integration – Enhancing participation in social, educational, and vocational settings.
  • Community Tenure – Supporting long-term stability and independent living within the community.
  • Behavioral Challenges – Addressing symptoms of Serious Mental Illness (SMI) or Severe Emotional Disturbance (SED) that interfere with an individual’s ability to function as a fully integrated member of the community.

Mental Health Rehabilitative Services may include:

  • Medication Management – Ensuring proper use of prescribed psychiatric medications.
  • Psychosocial Rehabilitative Services – Providing therapeutic interventions to improve social and emotional well-being.
  • Skills Training and Development – Teaching essential life skills for independent living.
  • Crisis Intervention Services – Offering immediate support during mental health crises to prevent hospitalization or harm.

At One Heart Behavioral Health, we are committed to providing comprehensive rehabilitative services that empower individuals to lead healthier, more fulfilling lives within their communities.
