Mental Health Targeted Case Management

Targeted Case Management (TCM) services are designed to support individuals within specific target groups. Under the Texas Medicaid Program, Mental Health Targeted Case Management (MHTCM) is available to individuals of all ages diagnosed with a chronic mental illness or a combination of chronic mental illnesses, as defined in the latest edition of the American Psychiatric Association (APA)’s Diagnostic and Statistical Manual of Mental Health Disorders (DSM). Eligibility for MHTCM services is determined through a standardized assessment process to ensure individuals receive the necessary care and support.

MHTCM services assist clients in accessing essential medical, social/behavioral, educational, and other supportive resources. These services are categorized into two levels: Intensive Case Management and Routine Case Management.

  • Intensive Case Management involves community-based support provided directly to the client or their Legally Authorized Representative (LAR), with or without the client’s presence. This service helps coordinate and secure access to essential care and resources tailored to the client’s needs.
  • Routine Case Management is primarily office-based and focuses on assisting the client, caregiver, or LAR in obtaining and managing necessary services that align with the client’s needs.
